The DCH Rehabilitation Pavilion at Northport Medical Center provides hospital-based inpatient rehabilitation and coordinated outpatient therapy services in one convenient location. With a 42-bed inpatient unit and a skilled interdisciplinary team, we help patients rebuild strength, function and confidence after serious illness, injury or surgery.
What is Rehabilitation at the Pavilion?
Rehabilitation at the DCH Rehabilitation Pavilion focuses on helping patients regain independence with everyday activities at home, at work and in the community. Our hospital-based inpatient rehab program offers around-the-clock medical monitoring, intensive therapy at least five days per week, and a structured environment designed for recovery.
Our outpatient rehabilitation program provides continued therapy after discharge or following a physician referral, allowing patients to keep progressing toward their goals while living at home.
Conditions We Treat
Our inpatient and outpatient rehabilitation teams care for adults and, in selected programs, patients of all ages with a wide range of conditions, including:
• Stroke and other cerebrovascular disorders
• Traumatic and non-traumatic brain injuries
• Spinal cord injuries
• Neurological disorders, including Parkinson’s disease and other movement disorders
• Complex orthopedic injuries and surgeries, including fractures and joint replacements
• Amputations and limb loss
• Balance disorders and gait problems
• Other medical or functional conditions that result in loss of independence with daily activities
Services We Provide
Our inpatient and outpatient programs may include:
• Physical therapy for strength, mobility and balance
• Occupational therapy for daily activities and home safety
• Speech-language therapy for communication, thinking and swallowing
• Specialized programs for amputee care and balance/fall prevention
We also use advanced tools, such as swallowing assessments and functional training equipment, to support real-world recovery
Our Rehabilitation Team
Your care is provided by an interdisciplinary team that may include a rehabilitation physician, advanced practice provider, rehabilitation nurses, physical, occupational and speech-language therapists, and case managers. The team works together with you and your family to set goals, monitor progress and plan for a safe transition home or to the next level of care.
Getting Started
Our admissions and care coordination team will review your needs, help with insurance authorization and guide you through the inpatient or outpatient intake process. If inpatient rehab is not the right fit, we will recommend another level of care.
