Request Medical Records

How to Request Medical Records

  • Once patients have logged into the MyDCH Patient Portal, they will see a Download Medical Record button under the Health Record section of the portal (right side of page).
  • Once they click the button, they will be prompted through the next steps.
  • The patient is sent an email with brief instructions when the file is ready for download.
  • If the system is unable to complete the request, the patient will receive a notification to contact Medical Records at 205-759-7565.

Other things to know:

  • The downloadable patient file stays on the portal for only 72 hours.
  • Patients may request their records as many times as they wish.
  • The medical record received by the patient only includes encounters dating from May 1, 2021. However, all SpineCare Center records are available.
  • Acute care records prior to May 1, 2021, will be available in the future.
