The DCH Diabetes & Nutrition Education Center is an education and resource center for people with diabetes and other nutrition-related health needs, as well as their families.  Our nurses and registered dietitian nutritionists provide one-on-one and small group education to help you manage your condition, understand your treatment plan, and make confident day-to-day choices about food, activity, and medications.

What Is the Diabetes & Nutrition Education Center?

The Center is a recognized diabetes education program that offers structured diabetes self‑management education and medical nutrition therapy.  Our team teaches practical skills such as blood sugar monitoring, healthy eating, carbohydrate counting, and prevention of diabetes complications, along with personalized nutrition counseling for a range of medical conditions.

Education is delivered through individual appointments and small group classes, so patients can ask questions, practice skills, and receive tailored support.

Who We Help and Conditions We Support

Our diabetes education programs focus on people who:
• Have Type 1 diabetes
• Have Type 2 diabetes
• Have gestational diabetes
• Are at increased risk for developing diabetes

Our nutrition education services support patients managing conditions such as:

  • Pediatric and adult weight management
  • Cardiovascular disease (including high blood pressure and high cholesterol)
  • Gastrointestinal disorders
  • Increased risk for diabetes
  • Sports nutrition needs
  • Food allergies and intolerances

The Diabetes Education program provides structured training in self‑management skills to help you control blood sugar, prevent complications, and improve day‑to‑day self‑care.  A physician referral is required to enroll.

Nutrition Education Services

Our Nutrition Education program focuses on helping you use food to support your health, while still enjoying what you eat. Registered dietitian nutritionists work with you to set achievable goals and create a personalized plan that fits your preferences, lifestyle, and medical needs

Nutrition education can help with:

• Heart health and cholesterol management
• Blood pressure and blood sugar control
• Digestive and gastrointestinal concerns
