About Us

Helping people on their way to living up to their full potential, that's what every professional at DCH Rehabilitation Pavilion at Northport Medical Center strives for every day. They are achieving functional outcomes comparable to national figures by combining their resources with a unique approach to caring.

Since 1984, longer than any other program in West Alabama, the Rehabilitation Pavilion offers several inpatient and outpatient programs dedicated to helping patients reach their optimal level of independence. These programs are the beginning of a return to independence.

Located on the second floor of Northport Medical Center, our facility meets all licensure criteria of an Inpatient Rehabilitation Facility as required by Medicare. The Rehabilitation Pavilion is accredited by the Joint Commission on the Accreditation of Healthcare Organizations.

The DCH Rehabilitation Pavilion provides professional rehabilitation services with a caring spirit, while promoting independence and fostering a continuous learning environment for our patients and families. We are committed to treating patients and families as individuals by addressing their personal needs for community re-entry. The delivery of excellent patient care is assured through the support and interdependence of each team member.

Hospital Based Program

When admitted to the DCH Rehabilitation Pavilion, you will continue to receive hospital services and close medical supervision while participating in the rehabilitation program. Every patient receives around-the-clock medical monitoring and response by our nursing department. Physicians check in on our patients daily, and our rehabilitation physician consults with other medical specialists as needed. A medical response team is on site and available during medical emergencies, so there's no waiting for the ambulance.

We provide professional medical services, many of which are not readily available outside of a hospital based program. Our services include:
  • Daily visits from Physical Medicine & Rehabilitation Physician/Nurse Practitioner
  • CRRNs, RNs, LPNs and PCAs on duty 24-hours a day
  • S.W.A.T. (Subtle Warning Assessment Team) on duty 24-hours a day
  • Intensive Care Acute Medical Care Units
  • Specialty Physicians
  • On-site dialysis
  • On-site radiology services
  • On-site pharmacy
  • Cardiac monitoring
  • On-site laboratory
  • Wound Care Specialists

Team Approach

Independence does not come easily. It comes through weeks of hard work, intensive therapy and team effort. At the DCH Rehabilitation Pavilion you will be the most important member of that team. Our method of Cooperative Treatment Planning has been in practice for more than 25 years. While you are participating in your rehabilitation program, your rehabilitation team will work together to provide you the education, training and motivation needed throughout your stay.

As specialists in the field of rehabilitation, we are acutely aware of the impact disabilities and chronic illnesses have on the lives of patients and their families. Ensuring that our patients and their families are active participants in the rehabilitation process means a lasting benefit to the patient, the family and his or her community.

Team Conference:
A Team Conference will be held with treatment team members each week to discuss your plan of care, goals, progress and discharge plan. This ensures collaboration and problem solving regarding your specific needs.

Family Meetings: Family meetings will be held upon request or as needed to review your goals and progress.  The team members are also available answer your questions or address any concerns that you or your family members may have.  The case manager will contact the family to schedule family meetings.

Patient/Family Education: Patient/Family Education is an integral part of the rehabilitation program and continues throughout the patients stay. This education will be provided to help you and your family better understand diagnosis, treatments, and what to expect when you return home. Information is provided in a very personal manner and done to help everyone involved with the patient. If your family is not available during the day to be at the hospital, a scheduled family education session can be arranged prior to the day of discharge.

A CVA Educational Class meets 4 days per week. The class is taught by rotating staff specialists. Patients who meet the class criteria are scheduled by the treatment team to attend this group. Each attendee will receive a stroke education booklet. Caregivers are encouraged to attend this class with the patient. The education focuses on warning signs/symptoms of a stroke, stroke prevention and compensatory strategies.

Facility Services & Equipment

The DCH Rehabitlitation Pavilion provides all of our patients with the best possible chance of improving functional and physical skills. Our 10,000 sq. ft. gym is equipped with state-of-the-art technology and plenty of space to practice your mobility. This gym provides an opportunity to practice activities that patients will encounter when they leave the hospital. Our gym is equipped with many features that help patients regain skills. This equipment includes:
  • An actual vehicle to help patients be as prepared as possible for their travels upon leaving the facility.
  • A full-size kitchen with stove, dishwasher, microwave, refrigerator and washer/dryer to  help improve everyday living skills.
  • Other features including stairs, a bedroom and weight machines for strengthening.
  • Lite Gait equipment for body-weight supported walking assistance.
We use the most current treatments and technology to help our patients regain skills. Some of the resources available to our patients include:
  • Captain's Log cognitive training software
  • VitalStim Swallowing Program
  • A bladder scanner to assist with bladder management
  • Assistive listening devices such as Pocket Talker
  • Driving and evaluation retraining program
  • Nintendo Wii gaming system to help improve visual-perceptual processing, postural control and funtional mobility
