Northport Medical Center has one social worker and 4 case managers on staff to assist with social services for acute med/surg and the DCH Rehabilitation Pavilion. Services are provided for any patient who requests a social worker, or who is referred to the department after meeting specific criteria. The department’s focus is centered on discharge planning needs, such as nursing home placement, arrangements for Home Health, home medical equipment or referrals to local, state or federal agencies.
The department provides counseling for patients coping with a new diagnosis, such as cancer, stroke, mastectomy, colostomy, etc. Social workers at the hospital also provide emotional support and guidance for parents who have lost a baby, or for those having a family crisis. Our social workers can assess financial eligibility when needed and advocate for patients and their family’s rights.
The department is fully staffed Monday through Friday from 8 a.m. until 4:30 p.m. One case manager is available on Saturday from 8 a.m. until 4:30 p.m., and one is on-call on Sunday from 8 a.m. until 4:30 p.m.
