A completed application packet for the DCH Residency includes the items listed below.

• Letter of interest
• Current curriculum vitae
• School of pharmacy transcripts
• Three recommendations
• Letters addressing candidate specific strengths/weaknesses preferred
• ASHP resident applicant recommendation form also accepted
• Professional writing sample:  newsletter, drug monograph, etc.

The application deadline is January 11, 2013.  Applications will be reviewed once all materials are received.  Interviews will be scheduled from mid-January through February.
