Fayette Medical Center Long Term Care provides skilled and intermediate nursing services on a 24-hour basis. Our facility has been serving the geriatric community of Fayette and surrounding counties for nearly 50 years. Our staff is dedicated to the quality healthcare that is provided through compassionate care, a homelike environment and a commitment to minister to our residents and families.  

Located adjacent to Fayette Medical Center, Long Term Care is currently licensed for 122 skilled nursing home beds by the Alabama Department of Public Health, and is certified for Medicare and Medicaid admissions, in addition to private-pay and LTC insurance coverage.  OUr location allows full access to outpatient services, emergency services and hospitalization at Fayette Medical Center.

Our services are designed to offer our seniors the highest level of care that contributes to their dignity, health, safety, and independence.  While no two individuals are exactly the same, our services are personalized to meet the individual needs of each of our residents.

Nursing Care
Fayette Medical Center Long Term Care provides long-term nursing care for our residents who need medical/rehabilitive and restorative care.  Long-term nursing care is probably the oldest, and most recognized, of all forms of senior care. Our long-term care is designed for seniors who require 24-hour nursing care, including medication administration, assistance with ADL's (Activities of Daily Living), eating, and other personal care assistance.

Our nursing care residences are designed for people who need medical/rehabilitive and restorative care.  Residents and their families are encouraged to participate in a care-planning assessment prior to or upon admission and are provided individualized care-planning options and health assessments. This resident-centered assessment helps determine the appropriate care and service that preserves each resident's  dignity, encourages independence and best enables freedom of choice. Constant communication with our residents and families help us to always improve our services.

Basic care services include assistance with eating, bathing and grooming, dressing, toilet help, mobility and transport, and other activities in which a resident may require help or supervision. Basic daily care is generally provided by Certified Nursing Assistants who help residents with their daily living activities.

Skilled care services include nursing care and related services for those who need medical, nursing, or rehabilitative services for the rehabilitation of injured, disabled or sick residents.  LPN's and RN's provide skilled nursing care services. We offer physical, occupational, and speech therapy services.

Social and interpersonal activities are provided for residents to interact daily.  Games, music, religious programs, arts and crafts, recreational and physical activities are provided regularly for mental and physical stimulation.

All of these services are provided by an attentive, caring and trained staff who are committed to the comfort and care of our residents.

Rehabilitation Services
Physical, occupational, and speech therapy rehab services are offered by Champion-Partners in Rehab.
